Acknowledgment of Risk

I understand that Brazilian Jiu-Jitsu involves strenuous physical contact, takedown, falls, chokes, joint locks, grappling and risk of serious injury or death. I am responsible for telling instructors about any medical conditions.

Assumption of Risk

I voluntarily assume all risks associated with participation, known or unknown.

Waiver of Liability  

To the fullest extent allowed by law, I release Jiu Jitsu Modern, owner, instructors, staff, and their agents from liability for injuries arising from ordinary negligence. 

Insurance

I certify I have adequate health or accident insurance to cover possible injuries.

Minors 

If under 18, a parent/guardian must complete the different waiver; please request from Tara Wolf.

Governing Law: This agreement is governed by New York law.
